Mail-In Enrollment

*ALL FORMS, DOCUMENTS AND REQUIREMENTS MUST BE INCLUDED IN YOUR ENROLLMENT PACKET BEFORE YOU MAIL IT IN FOR SUBMISSION.

1. Download the following:

   • BBI Student Enrollment Application​​

2. Provide Student ID Photos:

   • 2"x2" passport style photos​

     • you can get these made at your nearest Walgreens or create your student ID online​.​​

3. Include Transcripts/Proof of Education:​

   • Pick up a sealed, official transcript from your high school, ensuring that the seal remains unbroken

   • OR request for your educational institution to mail the official transcripts directly to the beauty school’s admissions office (address below)

   • OR Provide proof of your GED

   • OR provide a Homeschooling Certificate

4. Provide Proof of Age:

   • Include a copy of your driver's license​

   • OR a copy of your Birth Certificate

   • OR another valid form of identification that shows proof of your date of birth.

5. Enrollment Fee Payment: (Non-Refundable)

   • BBI Application Fee - $105​

   • State Board Licensing Fee - $5

     • Include a Check or Money order for a total of $105 payable to "Bloom Beauty Institute".​​

Once you have gathered ALL documents and requirements, mail your complete enrollment packet to:

1131 N. Desloge Drive #184 Desloge, Missouri, 63601.

After receiving your enrollment packet, our admissions team will review your application promptly. If additional information or documents are needed, we will notify you. Once your application is approved, you'll be contacted to discuss the next steps in your enrollment process.

​*Please note, enrollment is limited. Once our program reaches the maximum capacity, accepted applicants will be added to our waiting list.

*Enrollment Fees totaling $105 are non-refundable. Payment does not guarantee your enrollment in Bloom Beauty Institute's Cosmetology Program. Enrollment is subject to approval and will be processed on a first come, first served basis.
